About Thr1ve Thr1ve is a youth social prescribing service delivered by Chesterfield FC Community Trust. We provide short-term, person-centred, non-clinical wellbeing mentoring for young people aged 14–18 (or up to 25 with additional needs) across North Derbyshire. We support young people who may be struggling with low confidence, anxiety, social isolation, or low mood. Our approach is relationship-based and community-focused – helping young people connect with activities, opportunities and support that builds confidence, independence and belonging. About the Role We are looking for an enthusiastic, approachable Youth Wellbeing Mentor to join our Thr1ve team. This role blends one-to-one mentoring with community networking and partnership building. You will support young people to explore ‘what matters to me’, identify personal goals and take manageable steps towards improved wellbeing and independence. Key aspects of the role include: Providing one-to-one, strengths-based support. Building trusted relationships in a relaxed, non-clinical way. Supporting young people to access and settle into community groups and activities. Developing and maintaining referral pathways with schools, GP practices, CAMHS and community organisations etc. Networking across North Derbyshire to increase awareness and appropriate referrals. Capturing impact data to evidence outcomes. Who We Are Looking For We are particularly interested in candidates who: Have experience working with young people from similar backgrounds to those accessing Thr1ve support. Are comfortable building rapport quickly and working in a natural, approachable way. Can balance a relaxed style with clear professional boundaries. Feel confident networking and maintaining professional partnerships. Are organised, proactive and able to manage a caseload independently. Hold a full UK driving licence (non-negotiable due to community-based delivery). Professional Development & Support The successful candidate will receive social prescribing training, regular supervision and reflective practice. There are opportunities to undertake additional relevant training aligned to the role and personal development interests.
